Restaurant Ratings Most Violations: August 18 - 23

Ever wonder what goes on in the kitchen of your favorite restaurants?

We've gathered details about the 34 Tampa Bay restaurants that received the most violations during inspections conducted from August 18, 2014 to August 23, 2014.

Each restaurant is shown in the slideshow above. The total number of violations the restaurant received is shown in the plate on the left side of the slide. Some details from the inspection report are listed below the slide. If you click on the graphic in the slideshow, it will take you to the original state inspection report.

The 8 restaurants with the highest number of violations for the week are listed below.

Vallarta's Mexican Restaurant at 9212 Anderson Rd in Tampa received 56 violations - full report

State inspectors visited on August 19, 2014 and found the dishwasher handled soiled dishes, then handled clean dishes or utensils without washing hands. Raw animal foods not properly separated from one another, and toxic substance/chemical stored by or with food. No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.

The last inspection on March 4, 2014 the restaurant received 30 violations.

Rumba Island Bar & Grill at 1800 Gulf to Bay Blvd in Clearwater received 36 violations - full report

State inspectors visited on August 22, 2014 and found live, small flying insects in food preparation area, an employee touching ready-to-eat food with their bare hands. Potentially hazardous food such as crab, pineapple sauce, chicken and beef, held at greater than 41 degrees.

Follow up inspection on 8/25, received 11 violations.

China Gate at 12919 Walsingham Rd in Largo received 36 violations -

State inspectors visited on August 18, 2014 and found live, small flying insects in food preparation area, roach activity present as evidenced by live roaches found, raw chicken stored over cooked chicken. And toxic substance/chemical stored by or with food.

A follow up inspection on 8/19, received 3 violations.

Hayseed's Café at 599 Market Blvd in Webster received 33 violations  - full report

State inspectors visited on August 20, 2014 and temporarily closed the facility. A follow up inspection on 8/21 found 14 violations, and the facility was temporarily closed again. Another follow up inspection on 8/25 found 3 violations, including roach activity, and the facility remained temporarily closed. Follow up inspection on 8/25 found 2 violations.

When state inspectors visited on August 20, 2014 they found roach activity present as evidenced by live roaches found. All potentially hazardous foods in reach-in cooler cold held at greater than 41 degrees and toxic substance/chemical stored by or with clean or in-use utensils.


China One at 8747 Temple Terrace Hwy in Temple Terrace received 33 violations - full report

State inspectors visited on August 18, 2014 and found improper side-by-side separation of raw animal foods and ready-to-eat foods. Dead roaches on premises. Food debris accumulated on kitchen floor. Ready-to-eat, potentially hazardous food prepared onsite and held more than 24 hours not properly date marked.

A prior inspected on February 26, 2014 found 42 violations.


Square Plate Restaurant at 9131 US Hwy 19 N in Pinellas Park received 32 violations - full report

State inspectors visited on August 22, 2014 and temporarily closed the facility after finding roach activity near slicer table, prep table, on plastic wrap of opened biscuit tray in kitchen, 8 live roaches in stack of black plastic baskets on can shelf below microwave.

They also found an accumulation of dead flies and spiders in hall near back door. They also observed 38 dead roaches on black dry storage shelves in kitchen below pass thru window. Canned goods, clean plates, partially used biscuit package, condiment bottles, batter mixes are stored on this shelf. 1 dead roach on floor behind Pepsi glass door cooler.

The facility reopened on 8/23 when state inspectors found 11 violations.


Saigon Deli II LLC at 3858 W Waters Ave in Tampa received 31 violations - full report

State inspectors visited on August 18, 2014 and found live, small flying insects in food storage area, raw animal foods not properly separated from each other in holding unit. An employee touched face/hair and then engaged in food preparation. And an employee failed to wash hands before putting on gloves to work with food.

A follow up inspection on 8/19 found 7 violations.

Fung Garden Chinese Restaurant at 5118 Gall Blvd in  Zephyrhills received 31 violations  - full report

State inspectors visited on August 20, 2014 and found dead roaches on premises. The handwash sink used for purposes other than handwashing and the interior of reach-in cooler soiled with accumulation of food residue. Also, food was stored on the floor.

A previous inspection on March 5, 2014 received 22 violations.

Inspectors from Florida's Department of Business and Professional Regulation conducted the inspections. The department cited violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code.

While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary, according to the Florida DBPR website.
